1. Pre-Visit Checklist
Before your first entry into a Skilled Nursing Facility (SNF), ensure the following information has been processed:
- Credentialing: Confirm that your NPI, DEA, and California Medical License have been submitted to the SNF administrative backend.
- System Access: Verify that you have active credentials and electronic signature authority for the facility’s Electronic Medical Record (EMR) system (e.g., PointClickCare or MatrixCare).
- Logbook Link: Save the provided Jotform link. It is highly recommended to add this link to your smartphone's home screen as a shortcut for quick access.
2. Clinical Workflow
To ensure efficient service delivery, please follow this standardized process:
- Check-in: Upon arrival at the SNF, report to the nursing station to confirm the Daily Rounds List (the list of patients requiring consultation today).
- Assessment: Conduct psychiatric evaluations or follow-up visits as scheduled.
- Documentation (Critical):
- Same-Day Completion: Progress notes must be completed in the SNF’s EMR system on the same day as the visit.
- Electronic Signature: All notes must be electronically signed. Insurance providers will not reimburse for unsigned records, and the company cannot process your compensation without a confirmed signature.
3. Data Submission & Billing
Immediately after seeing each patient, submit a record via the Jotform link.
- Submission Details:
- Date of Service (DOS)
- Patient Initials
- Corresponding CPT Code (e.g., 90792, 99214, etc.)
- Deadline: All submissions must be completed within one hour of finishing your daily rounds.
- Compensation Basis: 1099 payments are issued based on a strict reconciliation between your Jotform submissions and the signed records within the SNF’s EMR system.
